Lttl Review: Is It Legit?

Lttl review: compounded semaglutide from $99 a month and tirzepatide from $149, the four pharmacies it names, refund terms, and which of its claims we could verify.

Last updated Oct 24, 2026
Quick answer

Lttl is an online GLP-1 program that prescribes compounded medication only: compounded semaglutide and compounded tirzepatide. Plans start at $89 a month. We rate it 9.1/10 (Excellent). It names the pharmacies that fill its prescriptions: RedRock Compounding Pharmacy, Health Warehouse, Precision Compounding Pharmacy and Triad Rx. It ships to all 50 states. Compounded drugs are not FDA-approved. No FDA warning letter is on file for Lttl as of Oct 24, 2026.

Lttl (pronounced "little") is an online program that prescribes compounded semaglutide and tirzepatide at one price for every dose. It suits you if you want a low flat price, month to month or on a longer plan, and don't mind that the prescribers aren't named.

The price is the draw. Compounded semaglutide is $99 a month month to month, or $89 a month on a 12-month plan. Tirzepatide is $149, or $139 on the 12-month plan. There is no membership fee and shipping is free.

We rate it 9.1 out of 10, tier Excellent. It is one of the cheaper compounded programs we track, it names its four pharmacies, and it has no FDA warning letter. Before you sign up, know that its plans are shorter than their names, its refund terms contradict its landing pages, and it names no prescriber.

Read the plan lengths before you pay. Each plan "month" is a 4-week supply, and the 6-month plan is a 24-week supply. Several of the site's headline claims, including a Forbes Health award and "125,000+" members, are ones we could not confirm.

What is Lttl?

Lttl is run by The Commerce Group Holdings, LLC. The Commerce Group lists Lttl among its brands, next to CBDfx, and its timeline shows a telehealth platform launched in June 2026. Lttl announced its launch on August 14, 2026. Its corporate office is in Woodland Hills, California, and it says that office is not a clinic.

Here is how it goes. You take a 2-minute online quiz, a licensed clinician reviews your answers without a video call, and if you are prescribed, the medication ships to your door. The care team answers by email, phone and in-account chat, Monday to Friday, 8am to 8pm Eastern.

Lttl sells compounded semaglutide and tirzepatide as weekly injections, lower-dose "microdose" plans of both, and other products it groups as longevity: NAD+, sermorelin, glutathione, MIC+B12, L-carnitine and methylene blue. Compounded GLP-1s are prepared by a pharmacy to a prescription, and they are not FDA-approved; the safety section explains what that means. To read about the drugs themselves, see our pages on semaglutide and tirzepatide.

Is Lttl legit?

Lttl is a real telehealth service with no FDA warning letter on file, and its terms name the company behind it and the four pharmacies that fill its prescriptions. Three things to weigh: it names no prescriber, several of its marketing claims don't match what we found, and its refund terms are strict.

The prescribers are not named. Lttl says care comes from "independent networks of U.S.-licensed clinicians and providers", and it names no medical group. Its "Medical Advisory Board" is a clinical pharmacist (PharmD), a family nurse practitioner (FNP-C) and a doctor of physical therapy (PT, DPT). The board "does not replace an individual consultation with your prescribing clinician."

The claims are the bigger gap. Most pages carry a banner reading "Forbes Health's Best Provider of 2026"; we found no Forbes Health article that names Lttl. The home page says "4.5★ from 125,000+ Members" and invites you to "Join 500,000+ others". Lttl's Trustpilot page holds 12 reviews.

The pharmacies are named. The terms list RedRock Compounding Pharmacy in St. George, Utah, Health Warehouse in Florence, Kentucky, Precision Compounding Pharmacy in Bellmore, New York, and Triad Rx in Daphne, Alabama, each with an address. Lttl's launch announcement says it works only with state-licensed 503A compounding pharmacies. One of the four has an FDA record; the safety section has the details.

As of October 24, 2026, compounded semaglutide costs $99 a month on the monthly plan, $96 a month on 3 months ($288), $93 on 6 months ($558) and $89 on 12 months ($1,068). Compounded tirzepatide costs $149 a month on the monthly plan, $146 on 3 months ($438), $143 on 6 months ($858) and $139 on 12 months ($1,668). The longer plans are billed in one payment for the whole term.

The price is the same at every dose, there is no membership fee, and shipping is free. The plan includes the clinician review, the medication, syringes and alcohol pads, and messaging with your care team. You don't need insurance.

The supply is shorter than the plan's name. The monthly plan is a "4-Week Supply", the 3-month plan 12 weeks, the 6-month plan 24 weeks and the 12-month plan 52 weeks. The plans page says the monthly plan is billed "$99/mo every month"; a Lttl landing page says it is "Billed $99 every 4 weeks". Billed every 4 weeks, the monthly plan is 13 payments a year: $1,287 for semaglutide or $1,937 for tirzepatide.

Each plan shows a crossed-out price, $249 for semaglutide and $339 for tirzepatide, with the "savings" counted against it. No plan on the page is sold at that price.

What to watch

Each "month" is a 4-week supply, the 6-month plan is 24 weeks, the terms refund nothing once the intake is submitted, the prescribers are not named, and the site claims patient counts and an award we could not verify.

Does Lttl have a discount code?

Lttl publishes no code to type in. A banner on most pages reads "Up to $140 off your first month · Code auto-applied", and it doesn't say which plans or how much applies to you. Some of its landing pages offer a free first month instead. Codes posted on coupon sites are not on Lttl's own pages. Check the total at checkout before you pay.

How does Lttl work?

The care is online and in writing. A clinician reviews your quiz and medical history, may message you for more information, and decides whether treatment is right for you. Follow-up assessments and check-ins happen through your account. Lttl doesn't say whether labs are needed.

The timing claims differ from page to page. The site banner says "Prescribed and shipped within 48 hours", and a landing page says "Clinician review within 48 hours". The shipping page allows 2 to 5 business days for the pharmacy to process an order, then 3 to 7 business days for delivery.

Lttl's terms also mention a prescription-only service for brand-name drugs "on a limited basis"; no brand-name drug is sold on the site.

Is Lttl safe? FDA status and warning letters

Lttl has no FDA warning letter on file, and neither does The Commerce Group Holdings. Nothing is open against it.

One of its pharmacies has an FDA record. Triad Rx, at the Daphne, Alabama address Lttl lists, received an FDA warning letter on February 5, 2019, for making sterile drugs under insanitary conditions. The FDA closed out the letter on May 3, 2023.

On July 31, 2026, the FDA issued a Form 483, a list of inspection observations, to Red Rock Pharmacy's site in Springville, Utah. Its observations include how the pharmacy tested its sterile filling process while filling semaglutide and tirzepatide lots. Lttl lists Red Rock's St. George, Utah site and doesn't say which site fills its orders. Health Warehouse and Precision Compounding Pharmacy have no FDA action on the lists we check, and none of the four is in the FDA's register of outsourcing facilities.

The company has an earlier FDA history in another business. The Commerce Group's chief executive is Ali Esmaili. In November 2022 the FDA sent a warning letter to Newhere Inc, doing business as CBDfx, addressed to him as its chief executive, about CBD products sold as foods, drugs and pet products without approval. That letter is not about Lttl or a GLP-1, and it does not count in our score.

The site's images show vials and a box printed "Lttl". Its footer says "Lttl does not manufacture medications" and that the product may look different.

The bigger question with any compounded program is the drug itself. Compounded GLP-1s are prepared by a pharmacy to a prescription. They are not FDA-approved: the FDA doesn't review their safety, effectiveness or quality before they're sold, and they are not the same as Wegovy or Zepbound. Formulas can differ from one pharmacy to the next, and some add ingredients such as vitamin B12. Compounding pharmacies are licensed and inspected by state boards of pharmacy; outsourcing facilities (503B) also register with the FDA and are inspected by it. The FDA can issue warning letters when either breaks rules on safety or sterility.

How to cancel Lttl

Cancellation
Cancel anytime
Money-back guarantee
A full refund if a provider finds you ineligible. Otherwise Lttl's terms treat an order as non-refundable once the intake is submitted, and nothing is refunded once it ships. You cancel by email.

You cancel by emailing the care team at hello@lttl.com. Cancelling stops future shipments and charges; orders the pharmacy has already processed can't be canceled or refunded. The terms say the deadline to cancel before a renewal is shown at checkout or in your account.

The refund rule is strict. The terms say: "Once the initial medical intake has been submitted, the order is considered processed and is non-refundable." You get a full refund if a provider finds you medically ineligible, unless you left out or misstated the condition that disqualified you. Refund requests must be made in writing within 28 days of the payment.

Lttl's pages don't agree on fees. Its refund page says consultation fees are "generally non-refundable once a provider has reviewed your information", while a landing page lists consultation fees under "Never charged" and says there is "No charge unless a clinician approves you". No consultation fee appears on any plan.

A 3-, 6- or 12-month plan is paid in one charge at the start. Only choose one if you expect to stay on the medication for that long. If you have side effects, talk to your provider.

How we scored Lttl

Our score combines seven checks. Each one counts for a fixed share of the total, listed below. Commission is not an input.

  • Price transparency: 10/10 (counts for 20% of the total score). Every plan's price, supply and amount billed is on a public page, with no membership or shipping fee, and our re-read matched every price. Read the plan details, though: each plan "month" is 4 weeks, the 6-month plan is 24 weeks, its pages disagree on whether the monthly plan bills every month or every 4 weeks, and every plan shows a crossed-out price no plan is sold at.
  • Cost and value: 9/10 (counts for 15% of the total score). We compare month-to-month prices: $99 for semaglutide ranks fourth among the compounded programs we track, against a median of $179.
  • Medication and pharmacy traceability: 9/10 (counts for 15% of the total score). Four pharmacies named with addresses, the pharmacy type stated, and the drug stated as compounded; none of the four is an FDA-registered outsourcing facility.
  • Clinical access and oversight: 8.6/10 (counts for 15% of the total score). Licensed clinicians, a stated review time and follow-up check-ins, but no lab policy. No prescriber or medical group is named, and the advisory board shown under "Meet the doctors" includes no physician.
  • Safety and enforcement: 8/10 (counts for 20% of the total score). No FDA warning letter against Lttl. One pharmacy, Triad Rx, had a warning letter in 2019 that the FDA has closed out, and the site shows a LegitScript seal that we could not confirm with LegitScript. The Form 483 and the CBDfx letter are not part of this check.
  • Terms, commitment and refunds: 10/10 (counts for 10% of the total score). No minimum term and a written refund policy. Note that the terms make an order non-refundable once the intake is submitted, while its landing pages say you are charged only if approved, and you can cancel only by email.
  • Availability and fulfilment: 10/10 (counts for 5% of the total score). All 50 states and Washington, DC, with free shipping.

This is the first score for Lttl. Confidence is high: we read its plans, terms, refund, process and product pages on October 24, and every price we track matched. No check carries an editor's adjustment. The method is on our methodology page.
