Quick answer

When the FDA inspects a 503B outsourcing facility, it nearly always issues a Form 483: 56 of 57 facilities with an inspection date on the FDA's 503B register had one, or 98.2%, on the register as of October 22, 2026. A Form 483 records an investigator's observations, not a final agency determination. Only 14 of those 56, or 25.0%, have a further action on file, such as a warning letter. The register covers all 503B facilities, not only GLP-1 compounders.

56 of 57 outsourcing facilities with an inspection date on the FDA's 503B register had a Form 483 issued following that inspection, on the register as of October 22, 2026.1 A Form 483 lists what an investigator observed; it records observations, not a final agency determination. The rate says less about any one facility than about the register as a whole: when the FDA does inspect, it nearly always finds something to document.

Key findings

1. A Form 483 is the outcome of 98.2% of these inspections

Of the 57 facilities on the FDA's 503B register with an inspection date, 56 have a Form 483 recorded against that inspection and 1 does not.1 It is not a claim that 98.2% of compounders are dangerous. A Form 483 is the document an investigator leaves at the end of an inspection listing what they observed. On this register, leaving one is the normal ending.

2. The rate has not moved as the program has sped up

38 of the 57 inspections on the register happened in the twelve months to October 22, 2026, and 37 of those produced a Form 483 — 97.4%, against 98.2% across the whole file.1 A finding rate that holds steady while the inspection count rises points to one standard applied to more facilities, not a crackdown.

3. 14 of 56 facilities went past the 483

A Form 483 is where an inspection ends, not where enforcement begins. The register's action column shows what happened next: 5 facilities have a warning letter recorded, 5 a regulatory meeting, 2 an FMD-145 letter and 2 an untitled letter.1 42 are recorded simply as open. So of 56 facilities with observations against them, 14 — 25.0% — have a further action on the file.2 For one of the 14, BayCare Central Pharmacy, the action on file is an FMD-145 letter from 2023, older than its 2026 inspection. That gap is why a single 483 should not be quoted as a finding of wrongdoing.

4. The register is a snapshot of the current inspection, not a history

Inspections on the file run from April 2025 to September 2026, distributed as 2025: 28 and 2026: 29.1 The register carries one row per facility holding its most recent inspection, so an older inspection is overwritten rather than added. The median inspected facility was last seen 0.8 years ago. Any rate computed here is therefore a rate across current inspection records, not across every inspection the FDA has ever conducted.

How this figure is derived

The numerator and the denominator come out of the same two columns of one file. From the FDA's outsourcing facility register we take every row with a "Last Inspection" date, which gives the denominator of 57; of those we count the rows where the register records a Form 483 issued.1 Facilities with no inspection date are excluded. They have their own page. Counting them here would measure inspection coverage, not findings.

A rate against the whole register would sound more alarming and say less, because it moves whenever the FDA inspects more facilities, whatever the inspections find.

The escalation counts are read from the register's own free-text action column, matched case-insensitively on the phrases the FDA uses there. A facility with no text in that column is not counted as escalated. Each facility counts once: where a warning letter close-out sits beside another action, the other action counts.1

Before quoting this number

  1. A Form 483 records an investigator’s observations. It is not a final FDA determination and not an enforcement action.
  2. The rate is the story, not any one facility.
  3. The register covers all 503B facilities, not only those compounding a GLP-1.
